chore: clean up CLAUDE.md — remove stale MCP entries, deduplicate infra context

Remove mcp__mariadb and mcp__proxmox-mcp from tools list (not configured
in .mcp.json). Replace duplicated infrastructure block with pointer to
global ~/.claude/CLAUDE.md.
