fix(916): cross-family state is per round, and the orchestrator reads cross_family before a verdict
A substitute that failed in round one said nothing about the tree that lands after round two, yet the flag was sticky and doomed the run; the xfamily string was never reset either, so clearing the stickiness alone would have let a stale "substitute ALSO failed" sentence into the PR body. Both reset at the top of review(). The harness runner is round-aware (ran per round, its own counter reset) and a two-round case pins the fix; restoring the sticky flag reddens it in both scripts. Step 4 of the mechanics page tells the referee to read cross_family, not only error, before posting on a rubric-class PR. fix(916): the Codex runner runs beside the lenses, only the fallback waits; head_sha described everywhere; the docs keep their thresholds
The runner builds nothing, so serialising it only added its wait to the critical path; the worktree-isolated fallback is what must follow the lenses, and the harness case now records lens count at the FALLBACK's start alone. setTimeout in the harness is globalThis.setTimeout (the .mjs lint config has ES builtins only). head_sha carries the same description in both scripts and every fixer/implementer prompt asks for the worktree HEAD, not a PR head. The mechanics page says why the cap stays at one after the serialisation and restores the 20%/10% RAM thresholds by key; the record says "several", not "three". fix(916): the cross-family review step follows the lenses instead of running beside them, and fix commits are a sha range
One .NET slot's review round ran two worktree-isolated reviewers at once — the correctness lens and, on a rubric change, the Codex fallback — and took swap from 6.8 GB to 10.8 GB in three minutes on the 16 GB host; three slots reached load 82. review() now awaits the lenses, then the Codex runner, then its fallback. The finisher's fix attribution is the sha range the fixer's report head advances (head_sha is required on every report), replacing a line-set difference over free text that listed all eleven #563 commits as fixes. The harness gains a case that records how many lenses were still in flight when the cross-family agents started (must be zero); moving the fallback back into the parallel batch reddens it in both scripts. The mechanics page and the standing prompt state the measured cap: one .NET-building slot at a time. fix(911): a round with no reviews is an error, the history binds each fix to the round it answered, and a harness executes the loop
A round in which every lens returned null read as a clean pass and would now have been quoted verbatim into the PR body; it is an error before the push, in the review loop and in the post-rebase round, which also gains the same blocking-or-should-fix filter; a fixer that dies or stops (no done) is an error too, the same test the implementer already gets. The history entry now carries the fix that answered that round and only the commits that fix added (a line-set difference against the previous branch log — a fixer that reformats or rebases mid-loop defeats it, which is why the finisher is told to read git show, not the list). An empty fix-commit set is described as "answered without a new commit" when a fix round ran, and as "round one was clean" only when none did. web/scripts/orchestration-workflow-loop.test.mjs compiles the committed script bodies with stubbed agent/parallel and pins eleven paths per script (22 tests). Measured: reverting the loop condition to blocking-only reddens six cases per script (every case that needs a should-fix round to reach the fixer); deleting any of the three zero-lens guards, the fixer guard or its done half, or the empty-fix sentence branch reddens its own case, in both scripts. web/vite.config.ts is untouched: it is pinned whole by test_image_build_delegates_the_spa_suite.py, comments included. fix(907): round two — a guard denial means a foreign worktree, required keys under every branch, a gated post-rebase round
The marker-overwrite instruction asserted an antecedent no agent can verify and, since same-session worktrees carry no marker, could only fire in another session's worktree; the scripts now stop and report. `ran` and `patch_changed` move into required keys of their own schemas so a missing field cannot read as a successful cross-family review or an unchanged patch. A blocking finding in the post-rebase round now returns an error like every other failure path. Each reviewer lens gets its own E2E port; the gate text travels with the reviewer brief. The standing prompt no longer contradicts the substitution the scripts perform; the record names patch-id, the mechanism the scripts use. The mkdir lock around scripts/e2e-local.sh serialised the launch, not the run (the launcher returns with the server up), and its stale-holder path double- acquired in 4 of 91 measured races; the launcher's documented conflict is its per-worktree wwwroot, so slots now run on their own port and the lock is gone with its inventory row. process.orchestrated-session records the two scopings the harness needed: a rebase pushed with --force-with-lease as the one sanctioned rewrite, and the referee as the only agent that ticks Done-when boxes. Scripts: required-arg guard, per-issue claim probe, reviewer fetch recipe, codex fallback to a cold review-only agent with the substitution stated in the PR body, rebase before the review loop with a patch-id check at the push, non-interactive squash recipe, Land phase. The single-issue kickoff stays as it is; this adds the layer that runs several of those under one referee. docs/handoffs/orchestration.md owns roles and sizing, one worktree per issue under ~/orca/workspaces/ersatztv/, the landing order with the review loop inside the worktree before the single push, and the merge through the consent hook. Three Workflow scripts encode it: a picker over scripts/select-queue.sh with two refuters, an issue-build pipeline (claim, recon, implement, gate, cold review with a cross-family runner for the rubric's risk classes, fix loop, finisher), and a resume pipeline for a paused branch. scripts/e2e-gate.sh serialises live-E2E across worktrees because e2e-local.sh refuses concurrent runs. `ci.cancelled-is-not-a-verdict` documented the run/job API, where cancelled is distinguishable. The endpoint a CI monitor actually polls — `commits/{sha}/status`, the per-sha view the merge gate reads — has no `cancelled` state and reports one as `failure`. The record now says to resolve the job-level `conclusion` before reporting a red.
The kickoff's push HARD CONSTRAINT is tightened from "a review has run" to "a CLEAN verdict, zero outstanding findings on the current tree", since #790's rounds 7 and 8 each still found a real mechanism defect and every earlier push auto-cancelled a live run.

.codex/ is generated by `codex exec` as a machine-local mirror of the .claude hooks. It is deliberately NOT tracked even though .claude/ is (17 files): its config.toml embeds a plaintext Gitea credential and absolute /Users paths, so committing it would leak the credential and would not be portable anyway. Ignoring it also unblocks scripts/refresh-shared-checkout.sh, which refuses on a dirty tree. Separately, docs/handoffs/rest-api.md carried the same credential inline; it now references $ETV_GITEA_BASICAUTH like every other doc. Two rules that were implied but not enforceable, both demonstrated by ersatztv#649 being implemented TWICE in parallel to completion on the same day. Subagents. The kickoff's HARD CONSTRAINTS already require parallel disjoint slices, mandatory independent review from a cold brief, and a named model/effort per dispatch — none of which is satisfiable without delegation. But nothing said so outright, and a generic client preamble of the form "do not use the Agent tool unless the user requested it" reads as a prohibition. Now stated plainly in both CLAUDE.md (always loaded) and the kickoff (pasted per session), with what to delegate and what to keep inline. Claiming. `in-progress` prevents duplicate PICKUP, not duplicate WORK — the record already said so, but step 3 told you to apply the label and nothing else. It now requires four checks first: an open PR whose body says `fixes #N`, a remote branch naming the number, a claiming comment predating the label (exactly what select-queue.sh's CLAIM? flag raises and deliberately leaves unresolved), and a fresh git fetch. Each fails differently; all four are cheap. Staleness. The second half of the #649 collision was reading origin/main once, at branch time, and not again across hours and four review rounds. A branch on a stale base computes its diff against that base, so `git diff origin/main` shows other sessions' merged work as DELETIONS and pushing it reverts them. Re-fetch before every push, rebase when it moved. process.parallel-session-claim carries the incident, including what worked: the merged implementation was better in one respect and the discarded branch's test coverage was salvageable, so diff the two before discarding yours. |

`pretooluse-merge-consent.sh` proves all three consent conditions at the moment the merge tool is called. With `merge_when_checks_succeed=true`, condition (a) is delegated to Gitea, which then merges whatever head is green at ITS merge time — while (b) Done-when and (c) the head-referencing verdict were proven against the head at SCHEDULING time. Every commit pushed in between merges unreviewed. The gate is not bypassed; it is satisfied against a snapshot that stops being true. Demonstrated as a controlled A/B rather than inferred, with a CI check left pending so Gitea waits as it really does: without a required verdict context: unreviewed commit B MERGED with it: same sequence REFUSED after reviewing B and posting it: merges — blocked, not deadlocked NOTE the anecdote in #622 is wrong and is corrected in the docs rather than repeated: PR #619 does carry `Review-verdict: MERGEABLE @ 02c82b35`, posted six seconds before the merge, explicitly re-reviewing the follow-up commits. #622 was filed off a lagging API read. The hole is real regardless, and structural — nothing FORCED that re-review inside the window Gitea would have merged in. This turns a property that held by diligence into one that holds by construction. The fix is the sha, not a smarter check. A Gitea commit status belongs to exactly one commit, so a status written for a parent cannot be inherited by a child that did not exist yet. `review-verdict/h10` becomes a REQUIRED status check on main: push a new head and the context is simply absent, which Gitea reads as not-passing (verified against 1.25.4: missing AND pending both block, and auto-merge re-checks the current head). It also covers merge paths the hook never sees — Gitea UI, raw API, another agent's session. - scripts/post-review-verdict.sh writes the comment and the status together so they cannot drift, and re-reads head after commenting: if a commit landed mid-flight it writes NO status and exits non-zero rather than retargeting the verdict at a commit nobody read. - .gitea/workflows/review-verdict.yml auto-passes the two exempt classes that would otherwise deadlock — Renovate-authored (platformAutomerge) and docs-only — and marks everything else pending. Exemptions are void when the PR touches .claude/, .gitea/, .husky/, scripts/ or docker/ci/. - The hook refuses to SCHEDULE an auto-merge unless that status is green on head, and no longer claims "CI green" on the mwcs path, where it never read CI. Two silent false negatives in the exemption path, both found by verifying rather than reasoning, both fixed at BOTH call sites (workflow and the hook's pre-existing docs-only carve-out): - The files endpoint caps at 50 rows and IGNORES a larger `limit` — PR #619 has 194 changed files and `?limit=100` returns 50. A single page saw ZERO protected paths there where the full enumeration finds ten. Both now page to exhaustion and withhold the exemption if they cannot complete. - A rename is ONE row whose `filename` is the destination, the source only in `previous_filename`. Verified live: `.gitea/workflows/renovate.yml` -> `docs/innocuous-note.md` presented as docs-only with no protected path visible. Both now read BOTH sides. Limits are documented, not papered over: base changes leave the head sha (and so the verdict) unchanged, and a PR editing the workflow is judged by its own edited copy — so PROTECTED is a guardrail against accident, not a tamper-proof control. fixes #622 |

Three independent CI/repo-hygiene fixes swept together; disjoint file sets. fixes #586 — E2E cleanup is scoped by PID, never a pattern-wide pkill - New decision record `testing.e2e-cleanup-scope-by-pid`. - docs/e2e-local.md states the constraint where a BRIEF-WRITER sees it (the #586 root cause was a delegation gap, not agent error). - scripts/e2e-local.sh: reviewed against e2e-ui.sh's trap lifecycle and deliberately does NOT adopt it — its contract is to hand a running instance back to its caller, so an EXIT trap would kill the server the instant the launcher returned (both callers use `OUT="$(e2e-local.sh ...)"`). Recorded. - Instead it gains what actually prevents the incident: an lsof pre-flight that NAMES a foreign listener's PID rather than letting Kestrel fail its bind and surface as "process N exited before becoming ready". - Pre-flight probes BOTH bound ports, and ETV_STREAMING_PORT now defaults to ETV_UI_PORT. Program.cs binds a second listener whose port defaults to 8409 independently of ETV_UI_PORT, so `ETV_UI_PORT=8420` alone still bound 8409 and died against a foreign holder — i.e. the documented escape hatch was a dead end that led straight back to the confusion behind the pattern kill. fixes #594 — ci-image-pin accepts any hex length - Length is a separate invariant from correctness: the resolve/staleness checks compare resolved shas, so an 8-char pin of the right commit passes green while matching NO registry tag, and all five container: jobs then die at image-pull with `manifest unknown` (reads like a registry outage). - Guard fails at the gate and prints the exact tag to use. Verified against doctored pins: 7 green; 6/8/10 red. - Uses a literal 7 rather than a derived `--short=7`: in a full clone git may widen an ambiguous abbreviation, demanding a pin ci-image.yml can never publish. Escape hatch documented inline. - Also fixes a pre-existing misdiagnosis: zero pins reported "MORE THAN ONE". - docs/ci-cd.md documents the 7-char rule and `git rev-parse --short=7 HEAD`. fixes #485 — .gitignore `core` silently ignored `*/Core/` files - A bare `core` matched any path component named `core`; case-insensitively on macOS that swallowed every `*/Core/` SOURCE dir, so new untracked files were dropped by `git add -A` while tracked ones stayed fine — a clean local build and a CI checkout that fails to compile. - Now `/core` + `/core.[0-9]*`, both anchored (an unanchored `core.[0-9]*` would re-introduce the same silent-exclusion class this fixes). - Verified by diffing the full ignored-file set before/after: identical, and the three real Core/ dirs are trackable without -f. Docs updated in-PR: docs/e2e-local.md, docs/ci-cd.md, docs/decisions/ workflow-process.md (+ regenerated catalog), docs/handoffs/chicorytv-issue-queue.md. The kickoff tells the orchestrator to route agents by capability, but that rule lived only in a prose paragraph. On 2026-07-25 a session dispatched two implementers (#436, #440) with `model` omitted, silently inheriting the orchestrator tier — while following every bullet in HARD CONSTRAINTS in the same session. The bulleted list is what functions as the checklist; prose above it reads as background. - HARD CONSTRAINTS: keyed routing bullet requiring the tier to be stated in the dispatch itself, so an invisible omission becomes visible output. Prose paragraph tightened to point at the key rather than restate the table (the kickoff is pasted into every session — duplication is a per-session tax, #542). - .claude/hooks/pretooluse-agent-model.sh: PreToolUse on Agent, `ask` when a committing agent is dispatched with no explicit `model`. Narrow by design — passes through read-only/recon types, `fork` (model override ignored by the tool), and any dispatch already naming a tier, because a gate that fires on every fan-out trains one-shot dismissal. `ask` not `deny`: routing is a judgment call with no derivable right answer, unlike the H6/H10 merge gate. - New decision record `process.per-agent-model-routing`; catalog regenerated. scripts/select-queue.sh ranked Renovate's Dependency Dashboard (#22) as an ordinary `priority: low` candidate. With an all-low backlog ordered by issue number, #22 sorted to the top and was proposed to every fresh session — a bot- rewritten status board whose checkboxes are commands to Renovate, not work items. A session trusting the selector's "trust the ordering" contract would either waste a pickup or make the exact undocumented judgment call the script exists to eliminate (this session hit it live). Drop bot-authored issues in the same jq pass that drops PRs/in-progress/parked: match login `renovate` plus the GitHub `name[bot]` convention so a future bot dashboard is excluded too. A bot's actionable output is PRs (already excluded); it never files a human work-item issue, so the whole class is never a pickup. Also make "scan for bundle-able siblings after claiming" an explicit kickoff step (new step 4) with a third bundle axis — shared label / adjacent subject — so a session sweeps small independent same-label issues together (e.g. this change's own #553 + #512 ci-cd hygiene bundle) instead of closing one at a time. Verified: selector no longer lists #22; genuine backlog issues still rank; the only bot-authored open issue is #22. shellcheck clean. Timothy asked why the kickoff handoff doc stores historical narrative when it should be instructions. It shouldn't — its own lore section is chartered as "STANDING workflow/orchestration rules only" with the why belonging in docs/decisions.md. But an inventory of every bullet against the decision corpus inverted the premise: only ~8 of ~38 were actually covered. 19 had no record anywhere and 11 more were half-covered, so that single file was the ONLY copy of the mandatory review rubric, the whole CI-triage vocabulary, the build-concurrency policy, the H12 session-end audit, and the plumbing-merge recipe. Pruning first would have destroyed them. So the records come first. New topic file docs/decisions/workflow-process.md carries 32 records (ci.*, process.*, testing.*) covering every NONE and PARTIAL the inventory found, including the Gitea `?milestones=` no-op bug whose only copy was the archived selector section this prune deletes. Only then the prune: HARD CONSTRAINTS and the lore section become one- or two-line rules, each citing the decision key that holds its evidence, and the 40-line "Archived — do not follow" section is gone. 636 -> 353 lines, with every cited key verified to resolve against the corpus. The aggregate corpus budget is re-baselined 4800 -> 5600 with the reason in the code: the corpus grew because knowledge MOVED into it, which is the system working, not drift. refs #542 |

A session was handed docs/handoffs/chicorytv-issue-queue.md pasted out of /Users/timothy/ersatztv while that tree was 81 commits behind, so it still described the queue protocol #520 retired the day before (read tracker command was ever run against that tree, so every existing "never read its HEAD" guard was irrelevant: a stale checkout serves stale FILES, and docs are what a kickoff depends on. Nothing broke only because selection went through scripts/select-queue.sh. The lore bullet on that tree already prescribed the shape of the fix for its earlier failure modes — "a design flaw, not a discipline failure; a check does not stay true" — so this removes the stale condition instead of adding another check. scripts/refresh-shared-checkout.sh fast-forwards the tree to origin/main and reinstalls web/node_modules when the lockfile moved. It is deliberately timid: it refuses and changes nothing when the tree is not on main, is dirty, is ahead, or is mid-rebase/merge, and it never switches branches, stashes or discards. A NO-OP is a normal outcome. Uses npm ci rather than npm install — the first version used install, which rewrote package-lock.json and left the tree dirty, i.e. the exact state the next run refuses on, so it would have disabled itself after one use. Exhaustive triage of the closed tracker ersatztv#237, whose 111 comments server-management#642 excludes from MemPalace ingestion (over the per-file cap) and #520 removes from startup. #524's premise was that facts living only in those comments would be orphaned and need curating into lifecycle records. Result: zero decision-shaped orphans. Every durable decision-shaped fact is already held by the decision corpus or by the individual issue the comment narrates -- which the exporter does ingest. Third review pass: MERGEABLE WITH NITS. Taking the one finding it asked for before merge, plus a doc nit. The cancellation filter added last commit had no token check, and it spans the whole Transcode body -- including every mediator send (ffprobe via CliWrap, media-server API calls, subtitle extraction, song-video generation). TaskCanceledException is also what HttpClient throws on its OWN timeout, so a real timeout in any of those was being downgraded from an ERROR with a stack trace to a routine "Terminating HLS session" Information line. Behaviour was unchanged (both arms return false) but the fault signal was lost, and this repo has been bitten before by "empty log != the event didn't happen". Now filters on cancellationToken.IsCancellationRequested, so only genuine caller cancellation is treated as a graceful teardown. Doc nit: the <exception> block said cancellation "is thrown"; it is only thrown when the token trips while the probe is in flight -- cancelling after it completes returns normally. Now says "may propagate". Declined the reviewer's optional suggestion to drain until a 0-return instead of reading exactly one byte: reading exactly one byte is what makes the guard safe BY CONSTRUCTION, since a server or proxy that answers 206 with a wider range than requested still cannot be drained unboundedly. 206-only was confirmed correct rather than extended to short 200s, since deciding "short" from Content-Length would reopen the unbounded path for a chunked or Content-Length-less response. Also records the operator's standing rule in the handoff lore: a lone `decisions.md append-only` red is a known infra flake -- do not investigate, rebase, amend or push to clear it; the operator reruns that job from the UI. I violated this earlier in this PR with a tidy-but-wrong "my entry is no longer at EOF" theory, and the rebase did not fix it -- the job went red again on a verified pure-append diff, which is the proof the red was never about the diff. The kickoff dispatches a cheap model to rank the backlog, and the lore then made the orchestrator re-derive its mechanical claims (deps, milestone tiering, priority order, in-progress state) because a small model kept getting them wrong. This pays that tax down: the mechanical checks are now CODE — GET /dependencies exclusion, LOCAL .milestone.state/review/priority tiering (never the no-op ?milestones= filter), (tier,priority,issue#) ordering, in-progress/parked/PR exclusion — leaving only two JUDGMENT flags (CLAIM?, UMBRELLA?) for a human/model to resolve. - scripts/select-queue.sh: fail-open on no-creds/unreachable; ~1.3s; shellcheck clean; UMBRELLA? computed in the jq pass (no per-candidate body fetch). - handoff kickoff: run the script FIRST; trust its deps/tiering/ordering, recheck only flags. - handoff lore: the three 'cheap selector unreliable → re-derive by hand' bullets kept as the EVIDENCE for why the script exists; the prescription is redirected to 'run the script'. Operator-requested after PR #405 pushed 5 times, orphaning live runs the operator had to cancel by hand. Batches every standing-lore correction this session produced into one commit (per the batching rule it adds). New HARD CONSTRAINTS: - BATCH PUSHES. Cancellation is impossible from the agent side on Gitea 1.25.4 — REST .../runs/{id}/cancel and MCP cancel_run both 404, and the web-UI route needs a session+CSRF that doesn't script. Only the operator can cancel, so an orphaned run holds a runner slot until it finishes. If you must supersede a live run, SAY SO. - TRUST THE GITEA BUILD QUEUE. Do not gate/throttle a push on host health; the runners were retuned for stability. Batch because you can't cancel what you orphan, not to protect the host. - BOM-CHECK touched .cs before pushing. The #311 gate is fix-as-you-touch, and it bit two sessions the same day (PR #405 ×6; #70/PR #402 ×19 via Python utf-8-sig writing BOMs back). Verify your detector — an od-based grep reported all-clean while 19 files were dirty. Corrects a claim I nearly published: `dotnet format --include` DOES work here. The apparent no-op was the SHELL — CI's mapfile is bash-only, zsh has no mapfile → empty array → zero files → exit 0. Run it under bash -c. THERE IS NO "main checkout" — the biggest correction here. /Users/timothy/ersatztv is a shared mutable working tree whose HEAD is whatever the last session left there. Its name lies, and it bit TWO sessions on 2026-07-17, both doing the obvious thing: one assumed main and committed onto the #604/CI session's branch 24s after that session's own commit; another read git log there and concluded main was "4 behind origin" — a phantom. Framed as a design flaw, not a discipline failure: "check git status first" appears to confirm the false assumption and then goes stale (it WAS on main at 12:46 and wasn't by 14:17). Worktree discipline itself is healthy — 10 feature worktrees. Diagnosing CI reds — three ways to misread one, all hit this session: - A KILLED job reports conclusion=failure, not cancelled. The tell is a log that stops mid-step with NO error and NO failure marker. A runner retune killed run 1006's migration + E2E on a BOM-only diff that couldn't break them. Log timestamps are UTC, host is UTC+2 — convert before correlating. - cancelled ≠ failure: a cancel is NO verdict, and a run marked failure may hold a genuine job failure from BEFORE the cancel. Monitors must count FAILED and CANCELLED separately. - "Unable to pull refs/heads/v4" is act refreshing its action cache and is followed by "Cloned …" — noise, not a cause. Grep the failure marker, not the word "error". An infra-shaped red (setup/cache step, before your code compiles) is not a code failure; don't file a CI bug off one sample. Also: the cheap selector's failure modes are wider than deps+priority — it also misses in-progress claim state and umbrella-vs-child. Docs-only. |

The H6/H10 merge-consent hook's satisfied path did a bare `exit 0`, which does not auto-approve a PreToolUse tool call — it only declines to block, so control fell through to the normal permission system and the raw MCP prompt still fired. A ready-to-merge PR was therefore confirmed twice (conversationally + a redundant mechanical prompt). Emit permissionDecision "allow" (new `grant` decision) on the satisfied and docs-exempt paths so the derived state IS the consent; deny/ask unchanged (fail-closed); non-merge methods keep the exit-0 passthrough. Docs: CLAUDE.md, kickoff HARD CONSTRAINTS, docs/decisions.md (append-only, pure insert). Two of the four non-hook #303 process follow-ups (the two docs items; the security scan and migration-on-prod-copy smoke are deferred to their own sessions): 1. Codex-skip rubric (kickoff workflow lore): an independent review pass is MANDATORY for diffs touching locks/concurrency, auth/security, API write-path handlers, or migrations, or >~150 changed C# lines; skippable only for a pure-SPA/docs leaf, and a skip must be stated + justified. Makes self-exemption an auditable claim (the correlated-blindspot net). 2. Live-E2E is now a STATED REQUIREMENT for API write-path handler changes: new "When live-E2E is required" section in docs/e2e-local.md + a decisions.md entry, formalizing the #229 lore bullet. The lore had grown into a changelog (~40 verbose bullets). Removed bullets whose substance lives in docs/decisions.md or api-conventions §7a/b/c (#251 post-commit None, #252/#259 reconcile, #253 PR3/PR4 concurrency, addTo layer, DTO nullable facts, disabled={saving}) and dead Blazor-parity process notes (parity verdicts, screen!=parity, unwired-endpoint gaps, #205/#206 docs-staleness). Condensed the survivors. Added a scope banner: this section is standing workflow/orchestration lore only; engineering decisions go in decisions.md, never here (they duplicate and drift).
